Coroner warns of risk of future deaths at mental health unit where patient was killed

What changed

A coroner has warned that further deaths could occur at Newham Mental Health Centre in east London unless serious staff and patient-safety failures are addressed. Hugo Flint Cahan, 34, was strangled by fellow patient Rolando Torres-Pena, 22, at the centre in January 2023; an inquest found that neglect contributed more than trivially to his death.

Why it matters

The concerns were not limited to one missed check. The coroner identified 14 problems, including staff sleeping or using phones during the night, missed patient observations, falsified records, delayed CPR, misleading information given to police and unauthorised two-hour breaks taken by staff.

That makes this a failure of basic safeguards, not simply a tragic incident between patients. The centre cares for acutely mentally ill men, so observation records, emergency responses and honest reporting are part of the safety system around vulnerable people.

The warning is especially serious because similar problems had been raised at a 2021 inquest. The coroner said measures promised after that case did not appear to have been implemented effectively.

East London NHS Foundation Trust says the failings were “wholly unacceptable”. One staff member has been dismissed and four others are under investigation. The trust says it has begun a major programme to improve inpatient services, but whether those changes have removed the risks remains unresolved.

The trust and NHS England must respond by 19 November. Their responses will show whether this becomes another set of assurances on paper or a clear account of how the ward will prevent the same breakdowns happening again.
